适应症
高血压危象(不推荐为一线用药)。
用法用量
初始肌内注射0.5～1mg，以后按需要每4～6小时肌注0.4～0.6mg。
不良反应
1．常见的不良反应有：倦怠、晕厥、头痛、阳痿、性欲减退、乏力、精神抑郁、注意力不集中、神经紧张、焦虑、多梦、梦呓或清晨失眠。2．较少见的有柏油样黑色大便、呕血、腹痛、心律失常、室性期前收缩、心动过缓、支气管痉挛、手指强硬颤动等。3．停药后仍可以出现的中枢或心血管反应有眩晕、倦怠、晕倒、阳痿、性欲减退、心动过缓、乏力、精神抑郁、注意力不集中、神经紧张、焦虑、多梦、梦呓或清晨失眠。精神抑郁的发生较隐袭，可致自杀，且可出现于停药后数月。
禁忌
抑郁症，尤其是有自杀倾向的抑郁症。
注意事项
1．对罗芙木制剂过敏者对本品过敏。2．利血平可以增加胃酸分泌和胃肠动力，慎用于有胃溃疡、溃疡性结肠炎或胃肠功能失调等病史者。3．利血平慎用于胆结石患者以防发生胆绞痛，慎用于过敏患者以防发生支气管哮喘。4．利血平慎用于体弱和老年患者、肾功能不全、帕金森病、癫痫、心律失常和心肌梗塞。5．利血平可能导致低血压，包括体位性低血压。6．治疗期间，可能发生焦虑、抑郁以及精神病。在服药剂量不大于0.25mg/日时，少见抑郁症发生；若之前就有抑郁症，用药可加重病症。一旦有抑郁症状立即停药；有抑郁症史的病人用药需非常慎重，并警惕自杀的可能性。7．当两种或两种以上抗高血压药合用时，需减少每种药物的用量以防止血压过度下降，这对有冠心病的高血压病人尤为重要。8．正在服用利血平的患者不能同时进行电休克治疗，小的惊厥性电休克剂量即可引起严重的甚至是致命的反应。停用利血平至少14天后方可开始电休克治疗。9．需周期性检查血电解质以防电解质失衡。10．麻醉期间用利血平可能加重中枢镇静，导致严重低血压和心动过缓。必须告诉麻醉师，事先给予阿托品防止心动过缓，用肾上腺素纠正低血压。11．利血平对化验的影响：以改良的Glenn-Nelson法或HoltroffKoch改良的Zimmerman反应作尿类固醇测定，可致结果假性低值；使血清催乳素浓度升高；短期大量注射使尿中儿茶酚胺排出增多，长期使用则减少；肌肉注射后尿中香草杏仁酸最初排出增加40%，第二天减少，长期给药排出锐减。
