适应症
1.皮肤粘膜的过敏:适用于长期的、季节性的过敏性鼻炎,血管舒缩性鼻炎,接触过敏源或食物而致的过敏性结膜炎,荨麻疹,血管神经性水肿,对血液或血浆制品的过敏反应,皮肤划痕症,必要时可与肾上腺素合用,作为本药的辅助剂。2.晕动病:防治晕车、晕船、晕飞机。3.镇静、催眠:适用于减轻儿童的恐惧感,呈浅睡眠状态。4.恶心、呕吐的治疗:适用于一些麻醉手术后的恶心、呕吐,也用于防治放射病性或药源性恶心、呕吐。5.术后疼痛:可与止痛药合用,作为辅助用药。
用法用量
口服 按体重一次0.5-1mg/kg,一日3-4次。或按体表面积一次15mg/m2,一日3-4次。
不良反应
异丙嗪属吩噻类衍生物,小剂量时无明显副作用,但大量和长时间应用时可出现噻嗪类常见的副作用。1.增加皮肤对光的敏感性,多恶梦,易兴奋,易激动,幻觉、中毒性谵妄,儿童易发生锥体外系反应。上述反应发生率不高。2.用量过大的症状和体征有:手脚动作笨拙或行动古怪,严重时倦睡或面色潮红、发热,气急或呼吸困难,心率加快(抗毒蕈碱(M)受体效应),肌肉痉挛,尤其好发于颈部和背部的肌肉。坐卧不宁,步履艰难,头面部肌肉痉挛性抽动或双手震颤(后者属锥外系的效应)。3.下列情况持续存在时应予注意:较常见的有嗜睡;较少见的有视力模糊或色盲(轻度),头晕目眩、口鼻咽干燥、耳鸣、皮疹、胃痛或胃部不适感、反应迟顿(儿童多见)、恶心或呕吐[进行外科手术和(或)并用其他药物时],甚至出现黄疸。使用栓剂时可发生直肠烧灼感或刺痛。4.心血管的不良反应很少见,可见血压增高,偶见血压轻度降低。白细胞减少症及再生不良性贫血则属少见。
禁忌
早产儿、新生儿应禁用。
注意事项
1.交叉过敏。已知对吩噻类药高度过敏的病人,也对本品过敏。2.对诊断的干扰:葡萄糖耐量试验中可显示葡萄糖耐量增加。3.下列情况应慎用:急性哮喘,膀胱颈部梗阻,骨髓抑制,心血管疾病,晕迷,闭角型青光眼,肝功能不全,高血压。胃溃疡,幽门或十二指肠梗阻,呼吸系统疾病(尤其是儿童服用本品后痰液粘稠,影响排痰,并可抑制咳嗽反射),癫痫患者(注射给药时可增加抽搐的严重程度),黄疸,各种肝病以及肾功能衰竭,Reye综合征(异丙嗪所致的锥体外系症状易与Reye综合征混淆)。4.应用异丙嗪时,应特别注意有无肠梗阻,或药物的逾量、中毒等问题,因其症状体征可被异丙嗪的镇吐作用所掩盖。
