适应症
盐酸多巴酚丁胺适用于：心脏血液输出量不能满足体循环要求而出现低灌注状态，需要采用强心剂治疗的患者；由于心室充盈压异常升高，导致出现肺充血和肺水肿的危险，需要进行强心治疗的患者。下列情况可造成低灌注状态。1、心源性A、急性心力衰竭。（1）急性心肌梗死。（2）心源性休克。（3）心脏手术以后。（4）药物引发的心脏收缩力下降，例如口肾上腺素能受体阻断剂过量。B、慢性心力衰竭。（1）慢性充血性心力衰竭的急性代偿失调。（2）作为常规口服强心剂、全身性血管扩张药以及利尿药的补充，对晚期慢性充血性心力衰竭进行暂时性的强心。2、非心源性的。A、由于创伤、手术、败血症或血容量不足导致急性低灌注状态，出现平均动脉压高于70mmHg且肺毛细管楔压为18mmHg或更高，并伴有对高容量及心室充盈压升高的反应不充分时。B、继发于机械换气且伴有呼气终正压（PEEP）的低输出量。在采用运动负荷试验诊断冠状动脉疾病时，盐酸多巴酚丁胺也许能用来替代体育运动。如同负荷试验中运动一样，当把盐酸多巴酚丁胺用于这一目的时，必须告知病人这种试验的潜在风险。此外，病人必须接受同样的严密监测，以进行标准的运动负荷试验，包括连续的心电图监测。
用法用量
成人常用量、将多巴酚丁胺加于5%葡萄糖液或0.9%氯化钠注射液中稀释后，以滴速每分钟2.5－10ug/┧给予，在每分钟15ug/┧以下的剂量时，心率和外周血管阻力基本无变化；偶用每分钟?15ug/┧，但需注意过大剂量仍然有可能加速心率并产生心律失常。【给药说明】用药前应先补充血容量、纠正血容量。药液的浓度随用量和病人所需液体量而定。治疗时间和给药速度按病人的治疗效应调整，可依据心率、血压、尿量以及是否出现异位搏动等情况。如有可能，应监测中心静脉压、肺楔嵌压和心排血量。
不良反应
当低灌注状态主要是由心输出量减低所引起且同时发生心窒充盈压升高的时候，输注盐酸多巴酚丁胺可能对压力的恢复起到帮助作用。在治疗急性低灌注状态时，当容量得到补充以及肺楔压或中心静脉压升高，而心输出量和动脉压没有升高时，盐酸多巴酚丁胺也许能改善输出量并对动脉压的恢复有所帮助。总之，当平均动脉压低于70mmHg且未出现心室充盈压升高时，可能存在着血容量不足，在给予盐酸多巴酚丁胺以前，需要用适当容量的扩容剂进行治疗。假如在给予盐酸多巴酚丁胺期间，尽管心室充盈压和心输出量处于适当的水平，而动脉血压维持在低水平或持续下降，此时可能需要考虑同时给予外周血管收缩药，例如多巴胺或去甲肾上腺素。实验室检查：象其它β2激动剂一样，多巴酚丁胺能够使血清钾浓度产生轻度的下降，但极少达到低钾血症的水平。因此，应当考虑对血清钾予以监测。
