适应症
用于纠正代谢性酸中毒，腹膜透析液中缓冲剂、高钾血症伴严重心律失常QRS波增宽者。
用法用量
1? 代谢性酸中毒按酸中毒程度计算剂量，静脉滴注碱缺失（mmol/L）×0.3×体重（kg）=所需乳酸钠（mol/L的体积（ml），目前已不用乳酸钠纠正代谢性酸中毒。 2? 高钾血症首次可予静脉滴注11.2%注射液40～60ml，以后酌情给药。严重高钾血症导致缓慢异位心律失常，特别是心电图QRS波增宽时，应在心电图监护下给药。有时须高达200ml才能奏效，此时应注意血钠浓度及防止心衰。3? 乳酸钠需在有氧条件下经肝脏氧化代谢成碳酸氢根才能发挥纠正代谢性酸中毒的作用，故不及碳酸氢钠作用迅速和稳定，现已少用。
不良反应
（1）有低钙血症者（如尿毒症），在纠正酸中毒后易出现手足发麻、疼痛、搐搦、呼吸困难等症状，是由于血清钙离子浓度降低所致。|（2）心率加速、胸闷、气急等肺水肿、心力衰竭表现等。|（3）血压升高。|（4）体重增加、水肿。|（5）逾量时出现碱中毒。|（6）血钾浓度下降，有时出现低钾血症表现。
禁忌
尚不明确。
注意事项
（1）浮肿及高血压患者，应用时宜谨慎。|（2）给药速度不宜过快，以免发生碱中毒、低钾及低钙血症。|（3）下列情况应慎用：|①糖尿病患者服用双胍类药物尤其是降糖灵，阻碍肝脏对乳酸的利用，易引起乳酸中毒。|②水肿患者伴有钠潴留倾向时。|③高血压患者可增高血压。|④心功能不全。|⑤肝功能不全时乳酸降解速度减慢。|⑥缺氧及休克，组织供血不足及缺氧时，乳酸氧化成丙酮酸进入三羧酸循环代谢速度减慢，以致延缓酸中毒的纠正速度。|⑦酗酒、水杨酸中毒、Ⅰ型糖原沉积病时有发生乳酸性酸中毒倾向，不宜再用乳酸钠纠正酸碱平衡。|⑧糖尿病酮症酸中毒时乙酰醋酸、β-羟丁酸及乳酸均升高，且常伴有循环不良或脏器供血不足，乳酸降解速度减慢。|⑨肾功能不全，容易出现水、钠潴留，增加心脏负担。|（4）应根据临床需要作下列检查及观察：|①血气分析或二氧化碳结合力检查。|②血清钠、钾、钙、氯浓度测定。|③肾功能测定，包括血肌酐、尿素氮等。|④血压。|⑤心肺功能状态，如浮肿、气急、紫绀、肺部罗音、颈静脉充盈，肝-颈静脉返流等，按需作静脉压或中心静脉压测定。|⑥肝功能不全表现黄疸、神志改变、腹水等，应于使用乳酸钠前后及过程中，经常随时进行观察。|（5）应用本品如遇有析出物时可加热溶解后使用。
