适应症
肠外营养药，能量补充剂。本品用于胃肠外营养，满足能量和必需脂肪酸的要求。
用法用量
通过外周静脉或中心静脉输入，一般情况下，输注脂肪乳应尽可能的慢。 1.成人： (1)最初30分钟内输入速度不应超过按体重每小时0.25～0.5ml/kg（约一分钟10滴），此期间若无不良反应，可将速度增至按体重每小时0.75～1.0ml/kg（约一分钟20滴）。每天脂肪乳输注时间不少于16小时，最好连续给药24小时。 (2)通过γ型接头，本品可与葡萄糖和氨基酸溶液经外周或中心静脉输入；在相容和稳定性得到确证的前提下，本品可与其他营养素在混合袋内混合后使用。一般情况下，本品不宜与电解质，其它的药物或其它附加剂在同一瓶内混合。除非另外规定或根据能量需要而定外，建议用量为：按体重一天1～2g脂肪/kg，相当于本品按体重一天5～10ml/kg。 (3)使用本品应同时使用糖类输液，糖类输液提供的能量应不少于40%。病人第一天的治疗剂量不宜超过250毫升，如病人无不良反应，随后剂量可增加。 2.新生儿：可递增至按体重一天3g脂肪/kg。
不良反应
直接与脂肪乳有关的不良反应一般分为两类：1. 即发型反应：呼吸困难，发绀，变态反应，高脂血症，凝固性过高，恶心，呕吐，头痛，潮红，发热，出汗，寒颤，嗜睡及胸骨痛等。2. 迟发型反应：肝脏肿大，中央小叶胆汁郁积性黄疸，脾肿大，血小板减少，白细胞减少，短暂性肝功能改变及脂肪过量综合症。有报道网状内皮系统褐色素沉着，也称“静脉性脂肪色素”，原因未明。
禁忌
脂肪代谢异常的病人（如病理性血脂过多）脂性肾病，严重肝损伤或急性胰腺炎伴高血脂症，则禁用本品。如病人患有酮酸中毒或缺氧，栓塞或休克则更应禁用。
注意事项
1. 在输入中/长链脂肪乳注射液时，应掌握病人血液循环中脂肪的廓清情况，血脂应在两次（天）输液之间清除。使用脂肪乳时间较长时，还掌握病人的血象，凝血情况，肝功能及血小板数等。脂肪乳输注过程中，血清甘油三酸酯浓度不应超过3mmol/L。2. 治疗过程中出现脂肪过量，应停止输注本品，检查血中甘油三酸酯水平回复正常后再使用。3. 在给患有代谢性酸中毒，严重肝损害，肺部疾病，脓毒血症，网状内皮系统疾病，贫血或凝血机能障碍，或有脂肪栓塞倾向的病人静脉输入脂肪乳时应十分谨慎。4. 太快输入脂肪乳会引起液体和或脂肪负荷过重，从而导致血浆中电解质浓度稀释，体内水潴留，肺水肿，肺弥散能力受损。5. 中/长链脂肪乳注射液为一次性包装，用剩的须丢弃，不可再用，如瓶内乳液出现油滴则不能使用。6. 本品贮存应避免冻结，冻结则丢弃不用。7. 蛋、豆类过敏者，可能对脂肪乳过敏。
