适应症
1.单纯疱疹病毒感染：用于免疫缺陷者初发和复发性粘膜皮肤感染的治疗以及反复发作病例的预防；也用于单纯疱疹性脑炎治疗。
2.带状疱疹：用于免疫缺陷者严重带状疱疹病人或免疫功能正常者弥散型带状疱疹的治疗。
3.免疫缺陷者水痘的治疗。
用法用量
本品用0.9%生理盐水或5%葡萄糖水稀释至至少100ml，使最后药物浓度不超过7g/L，否则易引起静脉炎。静脉滴注:1成人?①重症生殖器疱疹的初治：按体重每8小时5mg/kg，共5日；②免疫缺陷者皮肤黏膜单纯疱疹或严重带状疱疹：按体重每8小时5～10mg/kg，静滴1小时以上，共7日～10日；③单纯疱疹性脑炎：按体重每8小时10mg/kg，共10日；④成人急性或慢性肾功能不全者不宜用本品静滴，因滴速过快时可引起肾功能衰竭；⑤成人每日最高剂量按体重30mg/kg，或按体面积1.5g/m2。2儿童?①重症生殖器疱疹的初治：婴儿与12岁以下小儿，每8小时按体表面积250mg/m2，共5日；②免疫缺陷者皮肤黏膜单纯疱疹：婴儿与12岁以下小儿，每8小时按体表面积250mg/m2，共7日，12岁以上按成人量；③单纯疱疹性脑炎：每8小时按体重10mg/kg，共10日；④免疫缺陷者合并水痘：每8小时10mg/kg或500mg/m2，共10日；⑤小儿最高剂量为每8小时按体表面积500mg/m2。
不良反应
1、常见的不良反应：若注射浓度太高(10g/L)可引起静脉炎，外溢时注射部位可出现炎症。还可能引起皮肤瘙痒或荨麻疹。
2、少见的不良反应：注射给药特别是静脉注射时，有急性肾功能不全、血尿和低血压。
3、罕见的不良反应：注射给药时可能出现昏迷、意识模糊、幻觉、癫痫等中枢神经系统症状。
4、以下症状如持续存在或明显时应引起注意：注射用药引起的轻度头痛(常见)、多汗(少见)。
禁忌
对本品过敏者禁用。
注意事项
1、对更昔洛韦过敏者也可能对本品过敏。
2、脱水或已有肝、肾功能不全者需慎用。
3、严重免疫功能缺陷者长期或多次应用本品治疗后可能引起单纯疱疹病毒和带状疱疹病毒对本品耐药。如单纯疱疹患者应用阿昔洛韦后皮损不见改善者应测试单纯疱疹病毒对本品的敏感性。
4、随访检查：由于生殖器疱疹患者大多易患子宫颈癌，因此患者至少应一年检查一次，以早期发现。
5、一旦疱疹症状与体征出现，应尽早给药。
6?进食对血药浓度影响不明显。但在给药期间应给予患者充足的水，防止本品在肾小管内沉淀。
7、生殖器复发性疱疹感染以间歇短程疗法给药有效。由于动物实验曾发现本品对生育的影响及致突变，因此口服剂量与疗程不应超过推荐标准。生殖器复发性疱疹的长程疗法也不应超过6个月。
8、一次血液透析可使血药浓度减低60%，因此血液透析后应补给一次剂量。
