适应症
本品适用于敏感细菌所引起的成人与儿童的下列感染: 1.中耳炎、鼻窦炎、咽炎、扁桃体炎等上呼吸道感染。 2.慢性支气管炎急性发作、社区获得性肺炎等下呼吸道感染。 3.皮肤和软组织感染。 4.沙眼衣原体所致单纯性生殖器感染。 5.非多重耐药淋球菌所致的单纯性生殖器感染（需排除梅毒螺旋体的合并感染）。
用法用量
用法:本品为细粒剂,使用时将本品加入适量凉开水中,溶解后口服。 1.成人用量:第1日,一次0.5g,一日1次;第2～5日,一次0.25g,一日1次。或一次0.5g,一日1次,连服3日。衣原体引起的尿道炎或宫颈炎,1g单剂口服。 2.小儿用量: (1)治疗中耳炎、肺炎:第1日,一次10mg/kg(每日最大量不超过0.5g),一日1次;第2～5日,一次5mg/kg(每日最大量不超过250mg),一日1次。或按如下方法给药:体重在15~25kg之间,第一日0.2g,顿服,第2~5日,0.1g,顿服。体重在26~35kg之间,第一日0.3g,顿服,第2~5日,0.15g,顿服。体重在36~45kg之间,第一日0.4g,顿服,第2~5日,0.2g,顿服。 (2)治疗小儿咽炎、扁桃体炎:一次12mg/kg,一日1次(每日最大量不超过0.5g),连用5日。 (3)6月以下小儿的剂量及给药方法尚未建立。
不良反应
本药服药后可出现腹痛、腹泻、恶心、呕吐等胃肠道反应,其发生率较红霉素明显为低。偶可出现头昏、头痛及发热、皮疹、关节痛等过敏反应,过敏性休克和血管神经性水肿极为少见。少数患者可出现一过性中性粒细胞减少,血清氨基转移酶升高。
禁忌
对阿奇霉素或其它大环内酯类抗生素有过敏的病人禁用本品。
注意事项
1.轻度肾功能不全患者(肌酐清除率＞40ml/min)不需作剂量调整,但阿奇霉素对较严重肾功能不全患者中的使用尚无资料,给这些患者使用阿奇霉素时应慎重。
2.由于肝胆系统是阿奇霉素排泄的主要途径,故严重肝病患者不应使用本品,现尚缺乏这方面的具体资料。
3.在接受麦角衍生物类药物的患者中,由于同时使用某些大环内酯类抗生素而曾出现麦角中毒,目前尚无麦角与阿奇霉素发生相互作用的报道,但理论上仍存在这一可能性,因此阿奇霉素与麦角不应同时服用。
4.如同任何抗生素制剂一样,在本品疗程中,应对非敏感菌包括真菌所致的二重感染征象进行观察。
