适应症
主要用作叶酸拮抗剂（如甲氨蝶呤、乙胺嘧啶或甲氧苄啶等）的解毒剂。本品临床常用于预防甲氨蝶呤过量或大剂量治疗后所引起的严重毒性作用。当口服叶酸疗效不佳时，也用于口炎性腹泻、营养不良、妊娠期或婴儿期引起的巨幼细胞性贫血，但对维生素B12缺乏性贫血并不适用。近年应用亚叶酸钙作为结肠、直肠癌的辅助治疗，与氟尿嘧啶联合应用，可延长存活期。
用法用量
1．叶酸拮抗剂治疗后亚叶酸钙“解救”疗法：根据甲氨喋呤的血药浓度决定亚叶酸钙的剂量。一般静脉注射甲氨喋呤24小时后，采用本品剂量按体表面积9－15mg/m2，每6－8小时一次，持续2日，直至血中甲氨喋呤浓度在5×10-8mol/L以下。作为乙胺嘧啶或甲氧苄啶等的解毒剂，每次肌注9－15mg，视中毒情况而定。甲氨喋呤治疗后亚叶酸钙使用指导剂量：2．甲氧喋呤的过量补救：当不慎超剂量使用甲氨喋呤时，应尽可能及时使用亚叶酸钙进行急救；排泄延迟时，也应在甲氨喋呤使用24小时内应用亚叶酸钙。一般每6小时肌注或静脉注射亚叶酸钙10mg，直到血中甲氨喋呤水平低于10-8mol/L（0.01μmol）。出现消化系统反应（如恶心、呕吐）时，亚叶酸钙可胃肠外给药，但不可鞘内注射。治疗前后每24小时监测血清肌酐和甲氨喋呤水平。用药后24小时血肌酐超过治疗前50%或甲氨喋呤量大于治疗前5×10-6mol/L。或用药后48小时甲氨喋呤量大于治疗前9×10-7mol/L，亚叶酸钙的用量增加到100mg/m2，每3小时一次静注，直到甲氨喋呤水平低于10-8mol/L。3．叶酸缺乏引起的巨幼细胞性贫血：一般每天肌注1mg，尚无证据证明剂量增加疗效会增加。4．与氟尿嘧啶联用，用于晚期结、直肠癌（推荐以下两种联合用药方案）：1)缓慢静脉注射200mg/m2本品（不少于3分钟）后，接着用370mg/m25-氟尿嘧啶静注。2)静脉注射20mg/m2本品后，接着用425mg/m25-氟尿嘧啶静注。每日1次，连续5日为一疗程，间隔4周，用2个疗程；根据毒性反应的恢复情况，每隔4－5周可重复一次，并根据患者的耐受性调整5-氟尿嘧啶的剂量，以延长存活期。
不良反应
很少见，偶见皮疹、荨麻疹或引起哮喘急性发作等。
