适应症
适用于支气管哮喘，喘息型支气管炎、阻塞性肺气肿等缓解喘息症状，也可用于急性心功能不全和心源性哮喘。
用法用量
近期未接受过茶碱治疗的患者,静脉滴注首剂量（负荷量）按体重计成人或儿童为4.7mg/?,缓慢静脉滴注（输注速率不可超过20mg/分钟,滴注时间一般不低于30分钟）。维持剂量及输注速度见下表：静脉滴注茶碱剂量（急性支气管炎）1. 6个月至9岁儿童,12小时的维持参考剂量ｏ.95mg/［?（体重）・小时］。2. 9～16岁儿童及青年吸烟者,12小时的维持参考剂量0.79mg/［?（体重）・小时］。3. 不吸烟成年人,12小时的维持参考剂量0.55mg/［?（体重）・小时］。4. 老年病人及有肺疾患者 ,12小时的维持参考剂量ｏ.47mg/［?（体重）・小时］。5. 肝功能衰竭,12小时的维持参考剂量ｏ.39mg/［?（体重）・小时］。使用常规剂量时,若发生急性副作用,应立即停止给药5～1ｏ分钟或减慢给药速度。由于茶碱的治疗指数低,其代谢个体差异大,故用药剂量应个体化,并注意观察病人反应、肺功能以及药物血清浓度。对于慢性病治疗,测定用药3天的血清茶碱浓度以10～20μg/ml为宜。
不良反应
1. 常见的不良反应为：恶心、胃部不适、呕吐、食欲减退,也可见头痛、烦燥、易激动。2. 当静脉滴注过快或茶碱血浓度超过20μg/ml时,可出现毒性反应,表现为心律失常、心率增快、头晕、血压剧降、肌肉颤动或癫痫。由于胃肠道受刺激,可见血性呕吐物或柏油样便。
禁忌
对本品过敏的患者、未经控制的惊厥性疾病患者、心功能不全及急性心肌梗死伴有血压显著降低者禁用。
注意事项
1. 对诊断的干扰:本品可使血清尿酸及尿儿茶酚胺的测定值增高。2. 下列情况应慎用,并注意监测血清茶碱浓度:(1) 酒精中毒。(2) 心律失常。(3) 严重心脏病。(4) 肺源性心脏病。(5) 肝脏疾患。(6) 高血压。(7) 甲状腺功能亢进。(8) 严重低氧血症。(9) 活动性消化道溃疡或有溃疡病史者。(10) 肾脏疾患。(11) 年龄超过55岁,特别是男性和伴发慢性肺部疾病的患者。(12) 持续发热患者。(13) 使用某些药物的患者及茶碱清除率降低者。3. 静脉滴注时,应避免与维生素C、促皮质激素、去甲肾上腺素、四环素族盐酸盐配伍。4. 吸烟者、茶碱的肝代谢增多,需增加用药剂量。
