适应症
适用于各种原因的蛛网膜下隙出血后的脑血管痉挛和急性脑血管病恢复期的血液循环改善。
用法用量
体重估计低于70公斤或血压不稳定患者治疗开始的2小时可按照每小时0.5毫克尼莫地平给药（约为7.5微克/公斤/小时）。如果耐受性良好尤其血压无明显下降时，2小时后，剂量可增至1毫克尼莫地现约为15微克/公斤/小时）。体重估计大于70公斤测量宜从每小时1毫克尼莫地平开始约为15微克/公斤/小时）。2小时后如无不适可增至2毫克约为30毫克/公斤/小时）。若患者发生不良反应应减少剂量或停止治疗。严重肝功能不良，尤其是肝硬化时由于代谢清除害的下降，可导致尼莫地平的疗效和副作用，尤其是血压下降会要明显。在此情况下根据血压下降情况适当减量，如有必要，也应考虑中断治疗。每日总量20-40mg，7-10天为一疗程。尼莫地平输液经中心静脉插管用输液泵连续静脉输注，并经过三通阀与下列任何一种液体（5％葡萄糖、0.9%氯化钠、乳酸钠林格氏液、含镁乳酸钠林格氏液、右旋糖酐40溶液或6％的HAES聚氧-2-羟乙基淀粉），以大约1:4（尼莫地平：联合输液）的比例同时输注，也可与甘露醇、人血白蛋白或血液一起输注。严禁将尼莫地平输液加入其它输液瓶或输液袋中严禁与其它药物混合。麻醉、外科手术＼血管造乡术中应连续给予尼莫地平输液。输注尼莫地平注射液的聚乙烯管、联合输液管和中心导管应用三通阀连接。由于尼莫地平可被聚氯二烯（PVC）吸收，所以在输注尼莫地平时仅允许使用聚乙烯(PE)输液管。尼莫地平有轻微的光敏感性，应避免在太阳光直射下使用。如果输液过程不可避免暴露于太阳光下应采用黑色、棕色或红色的玻璃注射器及输液管，或用不透光材料将输液系和输液管包裹或遵医嘱。但如果在散射性日光或人工光源下使用本品10小时内不必采取特殊的保护措施。
不良反应
大量临床实践证明，蛛网膜下腔出血者应用尼莫地平治疗时约有11.2%的病者出现不良反应。|最常见的不良反应有：|（1）血压下降，血压下降的程度与药物剂量有关。|（2）肝炎。|（3）皮肤刺痛。|（4）胃肠道出血。|（5）血小板减少。|（6）偶见一过性头晕、头痛、面潮红、呕吐、胃肠不适等。|此外，个别病人可发生碱性磷酸酶(ALP)、乳酸脱氢酶(LDH)的升高，血糖升高以及个别人的血小板数的升高。
禁忌
尚不明确。
注意事项
（1）脑水肿及颅内压增高患者须慎用。|（2）尼莫地平的代谢产物具有毒性反应，肝功能损害者应当慎用。|（3）本品可引起血压的降低。在高血压合并蛛网膜下隙出血或脑卒中患者中，应注意减少或暂时停用降血压药物，或减少本品的用药剂量。|（4）可产生假性肠梗阻，表现为腹胀、肠鸣音减弱。当出现上述症状时应当减少用药剂量和保持观察。|（5）避免与β-阻滞剂或其他钙拮抗剂合用。|（6）稀释本品时，切勿用力振摇，以免析晶，如遇有析出物时可加热溶解后使用。|（7）使用时应避光。
