适应症
1.本品用于治疗对青霉素产生耐药而对依托红霉素敏感的细菌所致的感染。|2.临床上主要适用于耐青霉素的金黄色葡萄球感染，如肺炎、败血症、伪膜性肠炎、多发性疖痈等；亦可用于治疗链球菌、肺炎双球菌、军团菌、溶血性链球菌感染及白喉带菌者等。
用法用量
1.成人给药: |(1)常用剂量：一日750-2000mg,分3-4次服用。 |(2)军团菌病：一次500-1000mg,一日4次。 |(3)预防风湿热复发：一次250mg,一日2次。 |(4)预防感染性心内膜炎：术前1小时口服。 |2.小儿给药:每日每公斤体重30～50mg,分3～4次服用。
不良反应
1.用药后可出现恶心、呕吐、腹泻、中上腹痛、胃绞痛口舌疼痛、食欲减退等胃肠道症状,其发生率与剂量大小相关。 |2.偶可出现乏力、黄疸及肝功能损害,常于服药10-14日后发生。本药发生肝毒性反应较其它红霉素制剂多见,肝功能不全者慎用。 |3.过敏反应表现为药物热、皮疹、嗜酸粒细胞增多等,发生率为0.5～1%。
禁忌
1.对本药或其它大环内酯类药过敏者禁用。 |2.对本品过敏者禁用。
注意事项
1.患者对一种红霉素过敏或不能耐受时，对其他红霉素品种也可过敏或不耐受。|2.红霉素可通过胎盘而进入胎血循环，浓度一般不高，文献中也无对胎儿影响方面的报道，但孕妇应用时仍宜权衡利弊。|3.由于红霉素有相当量进入母乳中，哺乳期妇女应用时也应考虑利弊。|4.对诊断的干扰：红霉素可干扰Higerty法的荧光测定，使尿儿茶酚胺的测定值出现假性增高。血清碱性磷酸酶、胆红素、丙氨酸氨基转移酶和门冬氨酸氨基转移酶的测定值均可能增高。|5.因菌株对红霉素的敏感性存在一定差异，故应作药敏测定。
