适应症
用于心绞痛发作的治疗和预防，及对急性心肌梗塞合并左心衰和肺水肿病人入院前的紧急治疗。
用法用量
使用时，先揭开药瓶盖帽，喷射阀门处于上方，药瓶垂直，按压喷射阀门数次至喷雾均匀后则可使用。但若停用时间较长，则需再按压阀门至喷雾均匀后方可使用。使用时将喷雾嘴对准口腔，按压4揿，可达到有效剂量2.5mg。
不良反应
常见的有：由全位性低血压引起的眩晕、头晕、昏厥、面颊和颈部潮红；严重时可出现持续的头痛、恶心、呕吐、心动过速、烦躁；皮疹、视力模糊、口干则少见。极少数情况下可出现反跳性高血压、心绞痛、亚铁血红蛋白症。
禁忌
休克；低血压（收缩压＜11.7kPa）；急性心肌梗塞伴舒张末期充盈压低；对该药或其它有机硝酸酯类过敏者。
注意事项
1.交叉过敏反应，对其他硝酸酯或亚硝酸酯过敏患者也可能对本品过敏，但属罕见。
2.对诊断的干扰：
①血中硝酸盐类增多，变性血红蛋白也可能增加；
②尿中儿茶酚胺（肾上腺素和去甲肾上腺素）与尿中香草杏仁酸（VMA）值显著升高；
③用Zlatkis―Zak法测定血胆固醇可能造成假性降低。
3.下列情况应慎用或禁用：
①脑出血或头颅外伤，因本品可使颅内压增高；
②严重贫血患者应用本品时，可能加重心脏负担；
③青光眼患者禁用，因本品可增高眼内压；
④心肌梗死患者有低血压及心动过速者；
⑤梗阻性心肌病时可加重心绞痛；
⑥严重肾功能损害；
⑦严重肝功能损害可增加变性血红蛋白危险。
4.应用本品过程中应进行血压和心功能的监测，从而调整用量。
5.舌下喷雾用于缓解心绞痛急性发作，如15分钟内用过3次尚未能缓解，应即就诊。为了防止发作，劳动前5－10分钟，舌下喷服硝酸异山梨酯常可生效。
6.用药期间从卧位或坐位突然站起时须谨慎，以免突发体位性降压。
7.本品含90%乙醇，由于药液中含有乙醇，故在口腔内略有灼热感。
8.使用过频可使作用减低或失效。
贮藏
密封，室温保存。
